The medication administration forms are to be used for all prescribed medications and over the counter medications. NO medication will be administered in school without the proper completed forms. please see the MEDICATION ADMINISTRATION POLICY!
This form must be completed for all prescription/over the counter medication administered in school.
Administration of Epinephrine
If your child has a life threatening allergy and may require the administration of Epinephrine and/or Benadryl while at school, the following forms are required. Please complete the Administration of Epinephrine form and a Medication administration for for each medication
Medication will not be accepted in the nurse’s office without the proper completion of the below forms. All medication must be in the original containers. Epipens should not be expired and must have a prescription label which matches the doctor's order.

Middle School Sports Physical Information
A physical must be conducted and recorded by your physician or nurse practitioner on the New Jersey State Pre-Participation Physical exam form within 365 days of the sport's first day of practice. Please also have the athletic Eligibility Form completed. This form is what you will turn into the middle school.
